Alternative Spring Break 2009 Application
Selection Timeline
- Tuesday, November 3 at 4:30pm — Application due to SLIC Desk, Reeve 105 (Applicants must sign up for an interview at this time)
- Wednesday, November 4 — ASB Trip Interviews
- Friday, November 6 by 3:00 pm — ASB Trip Participants notified via e-mail
- Friday, November 13 by Noon — ASB Participants must pay $100 deposit
Failure to pay deposit will remove individual from the trip
Selection Criteria
- Selection will be based off the ASB Trip Application and Interview
- This trip is a joint venture between Reeve Union and Residence Life, as such, there are several outside factors considered during selection.
- Applicants must have a cumulative 2.5 GPA (Freshmen should list High School GPA)
- Having a record of the following will remove an applicant from consideration:
- Violent or Drug Crimes on CCAP
- Campus or Residence Hall Disciplinary Probation
- Illegal Drug Violations
- Violent Conduct Violations
- Sex Crimes
- Other Incidents that Raise Concerns

$100 due Friday, December 11
Final Payment due Friday, February 26

Application
If selected, I agree to:
- Pay the entire Alternative Spring Break fee, even if I do not attend for some reason.
- I acknowledge that if I use drugs or alcohol while on the trip, I will be sent back to UWO immediately - at my own expense.
- If selected, I acknowledge I must provide proof of insurance and other medical information as needed.
- My final payment for this trip will be due on Friday, February 26, 2010.
By filling out this form, you agree to allow the Alternative Spring Break Advisors to check your university discipline records, enrollment status, and Grade Point Average.
